They're literally not. It's pretty much like a little bro and friend's older brother situation. The guys grew up together and have a close bond. The kid feels like he has to take responsibility for the injury he gave the older guy when he was a kid. It's nothing bad, and the older character moreover treats the kid like you would your pet cat with "oh my god they're so cute" and cute aggression kind of energy. The kid has a one-sided crush, but that's normal for a child to do. Hell, when I was the kid's age, I had the FATTEST crush on Ricky Martin.
The two never actually get together, the kid "believes" he's the older one's boyfriend, but doesn't understand what it means, so it's played off as a joke kinda like he's just a friend that's a boy.

Why in chapter 21 when the kid sees the dude in a maid outfit does he say “something about it makes me feel hot and itchy” ? Why does a character say “isn’t he using it to masturbate” I know the black haired dude didn’t like that but why did the author feel the need to act that disgusting character to say that about an elementary school student. He shouldn’t have been let off with just an ear pull, if the author wanted to show that comments like that are gross the black haired dude should have addressed that in a serious and mature manner

The younger one has an attraction to the older one. That's it. The kid doesn't understand why his body feels that way and has no other way to describe it. Usually around that kid's age is when they begin teaching sex-ed (and sometimes discover porn), at least in my case that's what happened, so those kinds of topics can come up. Doesn't mean they really should, unless the kid's asking first, and it should only be with someone they trust that's like family imo. The older guy is pretty much a family friend.
