I find it interesting that Chae Woo is supposed to be this psycho killer, but he's really good at foreplay? Don't get me wrong, I love it, and I understand he changed, but I don't think you just become great at sex because of a new personality. I would imagine he'd have had sex for his own enjoyment with little regard for who he's with. Or **trigger warning** as another tool for violence.
Eating someone out is a skill that requires practice. Meaning, he would have been having to eat people out before his head injury. He did claim he was good at it when he first woke up, but it's hard for me to believe someone who is made out to be this horrible person before his memory loss would be good at pleasuring others.

If a man says he is good at something there are three possibilities. One, the dude is bluffing. Two, he’s over estimating himself. Or three, he’s actually good at it.
You brought up how he’s been presented as violent only. Therefore it’s unlikely he’s ever had sex or if he has, then he was violent and only in it for his own pleasure. People can be quite complex. A person can be violent outside but not in bed. A person can also be violent outside and in bed. We as manhwa readers need a bit more info regarding where he leans to be 100% sure.
I can agree that the second option you mentioned is very possible. He could be very much violent during sex, but we have to consider what kind of partners he would go for. Some chicks are into aggressive/violent sex. We also have to consider if he started off mega violent or grew into it.

I never caught any characterization that explained so I thought it was the same logic as him having a 6 pack every time he wakes up from a long coma or not needing a catheter during that time

Manhwa’s kill me with that type of logic. Muscle doesn’t work like that. He’d have it at the start of the coma, but if the coma lasts months/years, those abs/muscles will be gone. Also, depending on how long they are in a coma for, they’ll need to go through physical rehabilitation. Contractures occur from lack of movement. (Rehab can aide in bringing back mobility but if it’s severe enough, then it becomes permanent. It can be severe loss of mobility for the whole body or just certain parts.)
