Yes fl really died the first time when Cesare got back from the war and saw her being executed and shamed by the people. So Cesare found a way to bring her back but he had to endure 7 more of her deaths that all happened while it was raining and one of them happened on the bed at the brick house that’s why he’s sooo traumatized from seeing Eileen dead again. Also those 7 dreams made it difficult for him to distinguish reality from the dreams because CESARE is the one who had to kill Eileen each time that’s why he was strangling her at the brick house when it was raining cause he thought it was another dream and the real Eileen still wasn’t back but he knows it’s not a dream anymore
