No, they were confused, he really is human. What the story was saying is that both S classes are human, but are also different from other humans. Both the S classes and other people consider them so different they often refer to born S classes as monsters. What the chapter showed was that before awakening, he had a lot of violence inside himself, and he felt free to release that violence on the creatures escaping the dungeon- and in the process he awakened as a hunter with fire powers. But he's not a fire elemental himself. The fire elemental is the cute little salamander creature, and the fire person in the forge.
The novel and manga both don't say it directly, but I feel the author is inspired by how in real life families and society often reject nuerodiverse individuals, especially in cultures where fitting in is a requirement to be loved. People act more extreme in the story though because of the plot saying S classes are much more extremely different from others, and in comparison to fictional S classes regular nuerodiverse people are only a tiny bit different.
