Because Hestia had a child with Kael and due to the power struggle, Helios has the "Duty" to continue the imperial lineage which more important than anything else. That's why the concept of "love marriage" are not for imperial family.

It's not like Diana was forced to have a child. Helios let her choose between having a child or not but these two choices have consequences. Helios was under a lot of pressure to have an heir from the nobles because kael had a child.

They are not. The man he was sleeping with is an art teacher. I believe it confirms at the bottom of chapter 1 that the man is the painters art teacher, not the painters famous artist father.
