The author clearly portray real life doesn't always have good choice or bad choice cause life isn't that simple. Those ppl don't have crazy amounts of money to pay for their sick relatives. Hundreds of thousands of women every day turn to sex work to make ends meet. You live in a very privileged country if you don't see that happening. That's realism for you. Just look at the global south.
