This is their past life

They met in their previous life, referred as the “real world” as high schoolers. In this world Seungwon had abusive parents and a sister. It seems like Seungwon was very enamored with Seongjae, who later rejected/pushed him away to protect him from his ties to organized crime. It seems they ended up meeting in a building that collapsed before coming to the second world where most of the story takes place.
In this one, the two don’t meet until later. Seungwon has good parents and a brother.
His sister wrote the world into existence somehow, which seems to be the “original story” that Seungwon always references, and her, her brother, and her best friend got reincarnated there, but something went wrong with Seungwon, so he is constantly losing and recovering memories from both lives, and gets very sick when he becomes emotional.
We still don’t know exactly how his sister created this world, but she has been aware of what happened for a long time and kept it from Seungwon. The sibling doesn’t like Seonjae because he is associated with organized crime. This is likely why the original story she wrote in the first world had his actions lead to the death of Seungwon.
Seungwon has been trying to follow the story to die in his assigned “role” in the hopes of waking up back in the real world. He has some very severe mental health issues are likely caused by his fractured sense of reality and identity from the rebirth, as well as the repressed trauma of the building collapse which either killed him and Seonjae or came close.
There is a very strong sense of rejection, Seungwon feels that his family, himself, and the works around him are unnatural and not where he should be. He has been going through life not as a person who was reborn, but a person who is acting like a character to meet certain conditions to go home. He has a lot of depression, apathy, and ennui, so the risk of actually dying and not just waking up in his original world is not concerning to him.
I think we are going to learn more about how everyone got here, the origin of the second world, and how much his sibling actually knows about and influences it in the next few chapters.

I’m thinking he’s still alive and that’s why he has the fainting spells and the end if 70 is him realizing it and waking up in real world or fainting in real worldd and going back to story world. Who knows
