Reading the comments made me realize that people struggle to put themselves in other peoples shoes. Like... arranged marriages arent anything to sneeze at, it is literally you being used as a bargaining tool for your family and you have no say in it. Yes I agree shugu is the best, he is loving and caring despite his age, the best choice and I think Oto is trying to hide fiance from shugu because Oto doesn't want to inconvenience Shugu with the family dynamics he has been trying to escape. Like how would you react if you were living a peaceful life with a very good possible partner, the one of your child, and then reality comes running in and hitting you like a freight train and you havent been given much time to think about it?

I think the problem is also that people don't understand he was raised in a conventional mindset that he has to be this and that to be a perfect omega yadayada. He has very low self esteem and revolved his everything into raising Akane despite his flaws and the odds.
Shugu is YOUNG and a promising ALPHA, of course he's gonna think more of Shugu's future than his own feelings, also because HE DOES NOT FEEL WORTHY of Shugu. You could argue that "Yeah but that isn't his decision to make, it's Shugu's", but there hasn't really been an actual serious conversation on Shugu's intentions (literally they were one night stands, Akane was UNPLANNED pregnancy Oto chose to go through without Shugu's knowledge). At the current stage, Oto thinks HE'S A BURDEN and Shugu's just a good Alpha who wants to be responsible, but it wasn't really established that SHUGU WANTS TO BE THERE in Oto's mind. It's the thing they need to work on. Hopefully with this suspicious childhood friend appearance, they work on those creases.
But I hard agree that people are being too harsh on Oto. He was groomed to be the way he is, it's not something you can move on from easily (as someone from a conventional household as well).
Correction: Not one night stand, contractual/temporary relationship. Regardless, the bottomline is, their relationship wasn't supposed to be serious and they even planned to part ways eventually. Which is why I also think Oto doesn't want to tie Shugu... Yes I agree with what you've added, I dont come from a conventional family in the slightest so I do not have that perspective. But yes, the low self esteem, low self worth, that's a big part of this on top of the grooming.

In the notice, it says that this manhwa is connected to another manhwa that you can also read here. But it doesn't have very favorable opinions of it. I only read like 1 or 2 chapters so I can't really give a personal take. But from what I read in the comments it's not that good which is why I dropped it early.
