The entirety of Vol4 focuses on mpreg! We see ML getting pressured by his peers to have children, we see them facing the troubles of pregnancy, and we even see the twins! They have one boy and one girl: Doyoon and Seoyoon. It's a very satisfying side story volume!

The current arc in the manhwa is the last arc in Side Story 1, Vol 3 of the novel. Heesung meets Chiyoung's nanny, and basically it's all about Heesung trying to "cure" Chiyoung's separation anxiety, only to find out that he is in the same boat as him lol.
As for the mpreg in Vol 4, yes, Heesung will get pregnant. The entire vol 4 will be about mpreg. It's a very cute volume!
Chiyoung is being pressured to have babies since wolves usually start their family early, but he doesn't want to pressure Heesung. They eventually end up deciding to have a baby, but Chiyoung volunteers to be the one to get pregnant instead. They both go through fertilization, but Heesung's the one who got pregnant. Chiyoung is the one who experiences morning sickness though.
And we get twins! The wolf pups! Doyoon (boy) and Seoyoon (girl). Their nanny ends up being the dog hyung who works with Chiyoung since he knows both wolf and dog clans well. The pups even grow up in the novel up to five years old, and there's an arc where they both tried to get Heesung away from Chiyoung because they believe that he's been bullying their pup dad, even though they were actually having sex lol
Anyway, I bought v4 on Play Books, MTLed it and combined it with the existing epubs of the first three books. Enjoy~
