They are both psychologically insane, in one way or another. Sam- myeong's sanity was handled by a threat, mostly because he has always been driven by his right mindset and "what was expected of him to be". So he kept spiralling through a narrative and reality where he finally had to face making his OWN decisions, and trying to make them literally broke him, because all his conclusions lead him to commit "bestial acts" with the king.
