Well, he isn’t a doctor but he is a physiotherapist so still in healthcare care… so yeah, he should know better but since he lived his life as a beta I understand his confusion as well. Imagine you were born a man and suddenly you start having pregnancy symptoms (of course you can’t know they are specifically liked to pregnancy) so you get worried and later find out you are pregnant…. If its something thats not very common even us doctors would first considered other causes before giving a proper diagnosis, and from there exclude each suspicion based on physical examination, lab testing ecc...
