Like what the others said, it’s PTSD from when he was a child. His dad would grab the back of his neck from behind and threaten him. He once even held a knife (looked like a scalpel or box cutter or something) to his face.
I'm not sure if anyone would be able to find this with my poor hints but the art was majestic not a manga as it was coloured l. The story was something like the leads have known eo since high school/ college days, they found their sexual chemistry hot and started as fuck buddies. Then they started working, they lived together and one day one of them proposed that they might as well just date and they started to date. Yeah that's all I remember. It was smutty and was very realistic kinda very unique imo and it's not very popular.
