I was a bit confused too, until I read the entire prologue. This site only has like half of it but from what I think, and what a few ppl said below, he's on some sort of drug ("Switch") that changes ppls secondary gender. It was mentioned a bit in the prologue as well as the BL Guide on RIDI.
