He's had them since day 1 and they are usually always shown pretty well. Sometimes more prominent, but you can definitely see his deformed ears in most scenes

During when J was in his prime before the big incident there was only one kid he saved that he considered to be “his only success” because ml(who is the kid if you didn’t pick up on that) was the only one who gave J humanity when everyone else only saw and treated him as a hero without feelings. So since J held him so dear to him and felt like all his suffering was worth being able to save that one boy that’s how he got the name “my only success”
