It's frustrating, but some people are very dense... And sometimes it's difficult to see someone else's torment when he/she is trying with desesperation to hide it, specially from you.
In Kageyama's defense: Yashiro cut ties with him during some years (I don't remember how many). Yashiro is an extremely complex person, and very difficult to understand. And he makes a poker face when he is upset. And Kage still tried to be a good friend (he even works healing yazuka).
