This was a really beautiful manga. I just wish there was more to that ending. It would have been nice to see his old classmates' reactions or see if the relationship between Ishida and Nishimiya grew deeper than just barely holding hands for the first time. Overall I really enjoyed it just so much emotion. Good job mangaka!

The mangaka is Inariya Furanosuke, but I don't know the manga. It looks like it's related to her Maiden Rose serial.
The artist is Inariya Fusanosuke.
Their characters look quite similar to this.
