YES ABSOLUTELY!! (yapping session incoming...) I feel like the reason authors don't do this is because a lot of readers expecting a certain type of story are going to be disappointed at best or upset at worst, like people (mostly straight male readers I guess) getting into a shounen story only to find out they're gay (a similar thing happened with What Happens When the Second Male Lead Goes on Strike where I saw korean readers criticizing the author for making mc and the prince basically unofficially gay for each other, and that's a more of a shoujo so imagine if they did that to a shounen novel/manhwa), or readers shipping the FL in a rofan with a male lead only to find out that she's actually gay for her female best friend.
They would probably argue that the author should have tagged the story as BL or GL since the beginning, and at that point the twist wouldn't be a surprise anymore.
I mean I personally love these kind of twists, and I know many others do, but a lot of readers (maybe more the korean ones) would insult or at least get upset with the author and that's what they want to avoid :/ I guess they want to play safe

The best example i can give is something from Rimuru where he basically create his own nation of monster or from "i wil become head of this family" where Tia trying become head of her family, or something like from the "i become 9999level demon lord" where Raina become basically hero of north and well defacto duchess of the village doing her best to make sure the village Prosper and become big etc.
