The whole struggle Sakura was facing is that he's a gay man who likes girly things and wants to be treated like a princess if he was trans we'd see him struggle internally with his gender with the way he presented himself it's very clear he's not trans he thinks to himself hed be more accepted if he was a girl his hobby would be "normal " ik a lot of queer kids have thought about this too its not that he wants to be a girl cause hes trans its the fact that he would be accepted if he was born a girl infact hes ok being a middled aged old man he doesnt have gender identity issues there are many great manga with trans rep but this one isn't it . (& yes guys can also want to be princesses it doesn't mean wanting to be girl in Sakuras case it means wanting to be pampered loved having a prince charming adore you) Men can like extremely feminine things too & that was the point of this story it's normal & that's what the story was trying to portray.
