I read this so many times already, but I never understand, can someone explain it to me. Why didn't Rum recognized Mono, when he to visit, during hospital scene?

OK so Mono is usually this smaller looking chibi guy, but when he used his full powers to rescue rum and ginger he came into his true form, as a much larger more manly angel wearing different clothes. Granted to us he doesn't look all that different, but I guess to the characters he's different enough from how they usually know him to be surprised.
Full fledged adult seems to be referring to having "moved into adulthood" which was referenced as having had sex before. But can't guarantee that one
