He's aware that he's part of a story. He knows he's basically being forced into acting a certain way and doing certain things because that's what the author decided he would do in a shitty BL story. Since the world he lives in is entirely based on that shitty BL story, he can't change his destiny on his own. Basically he has no freedom to do what he wants as long as everything happens the way the author describes it. However, now that our MC is there, someone who shouldn't have been part of this story and therefor alternated the plot, he has the opportunity to change things. The MC was never supposed to be the hero, but his actions can greatly influence the way the story goes.

Or at least that's how it usually goes in those kind of stories. Characters being forced into doing things they don't actually want to do by some omnipotent power (the author of the story their world was based on) but then someone who wasn't supposed to be there shows up (the MC) and that can shake things up enough to finally allow them to live the life they want to live.
I usually get bored of those kind of isekais halfway through the story and then never finish them so I don't have much to recommend here. However, "Beware of the Villainess" does have a similar approach to the genre as this one (without spoiling too much). It's not BL though and has no sex scenes but it's amazing. Melissa (the MC) is one of the best female MCs I have ever encountered and the male & female lead are both great. The beginning is a bit slow but that's pretty much the only complaint I have.
