Without knowing the BDSM world, trying to understand this manga would be difficult. I am a practicing masochist - I actually like being beaten and humiliated - and am part of a local group. I've been into BDSM for over 25 years and part of this group for 15. The best way I can explain it would be this: Do you know why the things that turn you on the most, turn you on? Most people answer either "no" or "it feels good". For a masochist pain and humiliation "feel good". There is no rhyme or reason, it just does. I can tell you that masochists tend to have a high pain threshold - at least old ones like me... LOL. Pain and pleasure release the same hormones. So, it really isn't surprising that the two sensations can work hand in hand for some people, or one can even take the place of the other. In this story, conflict arose when the protagonists couldn't compromise. Matsumi(sp) was afraid of the violence and wanted to have "normal" sex and Tobito craved the violence and couldn't enjoy "normal" sex. This is common among young people who practice BDSM. It is also a common theme among young couples who engage in "normal" fetishes - foot fetishes, clothing fetishes, etc. Where does the fetish end and the loving relationship start? This story depicted this perfectly.

Well, you've asked a great question and I have to give a lame answer. It depends on the person. How they view the violence in BDSM is key. If they are introduced and guided carefully, they can come to terms with the "violence" not being violent at all. Rather it would be seen as an expression of love for their partner. Unfortunately, this is not the case often enough and relationships are lost. Most young practitioners - not just in age but in time with BDSM (a 50 year old doing BDSM for a year is "young") - tend to rush too quickly and scare their partner or themselves. In BDSM to rush is to fail. As the couple in the manga found out. When they stepped back and realized that the relationship was the important aspect and not the S&M, they were able to enjoy each other in ways outside of S&M... as depicted by the uke kissing the seme sweetly. Though it's not explicitly shown, I can imagine the S&M aspect being much better and under control as well.
I know many BDSM practitioners who started out scared or unsure and very much like the seme here. With experience and guidance they have become perfect BDSM partners for their lovers/spouses. It all depends on what the person wants to make of the relationship. Do they want to fulfill their partner's desires? Are they willing to open their heart and mind to things they don't yet understand? Are their feelings for their partner deep enough to try something scary in order to please them? These answers are what determine if the relationship will last.
