Well not like anyone's going to care but this is me but younger and with a bit of pimples (trypophobia, the only pimple selection are more than what I have and trypophobia). Set an alarm the day before the due date of your assignment. I can't say that I can relate to your missing works but I can relate to your procrastination. I would always cram at the last hour or sometimes I would totally forget to submit assignments since I've been too focused with my laziness (reading stuff, watching netflix, games, etc).
