What's the worst choice you can make in character design?

The obvious stuff like stereotypes and oversexualization out of the way.

If you have a harem and all of the characters have similar haircuts and faces (minus if the characters are related), congrats I fucking hate them all.

Staring off with very complex outfits and overall character designs and then falling off gradually, id rather consistency than the artist burning themselves out reply
