I don’t think it’s ever mentions a mental illness? I honestly thought it was more leaned towards him being a sex-addict? I still haven’t gotten the chance to reread this, so I’m probably wrong.
I don’t think he’s a sex addict and I do not have any qualifications to diagnose someone but I assume maybe PTSD, anxiety, or likely depression???
He seems to be dealing with a lot of trauma. At the very least, his ex boyfriend was a creepy and obsessive stalker, so now he struggles to be able to be in any romantic relationship with anyone. His trauma resurfaced when he saw the Seme in the bar.

Yes!! I agree with this. It makes sense, and I had honestly forgotten that he was in a previous relationship.
They don't mention it much in the story so it makes sense that you forgot, but it's the main reason he refuses to date the Seme. Since his ex left him traumatized with his stalker and obsessive behavior, he freaked out when he saw the Seme at the bar. He thought that the Seme was also stalking him. The trauma caused by his ex resurfaced and i think that's the mental illness he was referring to
