this is more than what meets the eye. sure, we see Hibiki as a woman then it ends with a woman. but the content in itself embraces Hibiki's male identity, both in psyche and physical trait. As a woman, they have exhibited masculinity. As a man, they have manifested feminine temperament. When Hibiki has thought of marrying Hiroto, both their male and female face are represented.

for me, we are too deceived by what we see hence we feel disappointed that Hibiki is a woman in the end. isn't that unfair for Hibiki's female counterpart as well? Seeing a man and a woman in public or as a end credit doesnt devalue the well-intended message and journey which Hibiki and Hiroto had undergone. Hibiki being in a womna's body doesn't mean they are no longer a man. It's more important for Hiroto to have accepted their sex than have the world see a homosexual couple.

Regarding birth, the couple didn't show any intention to have a child so I can shrug that off. However, basing on the doctor's info about hormone imbalance, it could mean that Hibiki could prolong their female body if demanded by pregnancy just like how women no longer menstruate during pregnancy.
