okaaay, MTL was very confusing so this is what I understood when I read it. I just jump at chapters so.. don't expect too much.
Remember MC has been visiting psychiatrist and having hypnosis sessions. It was explained during that time MC receives memories from his other personality. After he gains everything and every bit of information, it was said that MC had been thinking of another alter of themselves in it. Like how the third speculation of the post here says.
MC is basically using people he sees or know as the characters. His previous parents are real too coz at the end of the chaps he wanted to find them that was the confusing part honestly or MTL just sucks and my brains interpretation is wrong. LMAO
But honestly this is one is so interesting how the author will really explain this one, I hope it's not confusing as the MTL. Imma just wait for the English translation.

3. Or he just really does have DID (dissociative identity disorder) and thinking of himself as the actual character in the book he read.
Which is pretty weird cause i just learned yesterday it's a real thing where people with DID can literally think of themselves as another alter of a fictional character which is called a fictive interacting with other fictional characters? I don't know how that works but it's really complicated
