I really do NOT believe that junpei is the one reading the letter. satoko is his whole world, she's given him a purpose in life, to love her and stay with her. there's not a fucking chance that man is not choosing to die with her when her time comes. the kotaro theory makes more sense, he's gonna have another purpose in life even if sotoko is no longer alive, because he's surely ending up with that girl
Knowing Shinpei, if he doesn't turn into a completely new person, he might die with Satoko. In that case, what if the old person in the beginning is Kotaro? That will be less sadder if that's it. And it would be nice if after reminiscing, he gets back to his home with his own loved one.
