Shuraka is circumcised. Tei is not. That's basically it.
Shuraka had his foreskin removed and is circumcised
Tei still has his foreskin and is uncircumcised
Thats why they look different
It's called circumcision. For religious or community reasons this can be done. It's when the skin around the shaft, foreskin, is removed. Ml is circumcised but Mc is not.

I would say it is a cultural thing. Im from northern Europe country and circumcision is 99.99% not done here. I cant be 100% sure but I have never in my 42 years of life heard its done here...ok I googled it, its only allowed in some medical cases, otherwise its basically illegal and no doctor would do it. I saw circumcised dick first time irl last year when I satrted dating a korean guy...my first reaction was, did it hurt?
