Yup. The plot is the dick doctor is in love with large dicks. The doctor leaves him a note confessing to what he did. And so this gangster finds him and makes him pay 10 fold. Because that is what gangsters do when someone crosses them.
So yeah. The doctor is paying off his "debt" with his body, but it's not like Doc hates it. He just hates gangsters specifically.

Yeah that part I feel was idk… it’s hard to get past that cause wtaf, ur a LICENSED DOCTOR! and you essentially raped a patient while they’re under anaesthesia MULTIPLE TIMES no less!!!.
