well probably because he feels disgusted with himself, because he doesn't want them to hurt seunghyun, because he's suicidal, because he doesn't want to drag seunghyun down with him, because he's been through a traumatic experience for so long that the help he's about to receive feels like a dream, because he knows those evil guys won't ever leave him alone, they have a lot of members, and if one died, another will come, and all of this miserable thing will unfortunately continue until either the whole gang perished or Yuta himself dies, and I disagree with people who said "if he wanted to die, he should do it !" like wow, maybe just don't say anything at all ? he's bound to die anyway (if he doesn't want to proceed with the surgery), so why would he do something as painful as killing himself ? he's suicidal, yeah, but he's already been through so much pain, and is just waiting for the universe to do its job
