am i so dumb i don't even understand the plot and who's who

lol same. I saw a novel spoiler awhile back and I still feel kinda lost. I think I need to let this one marinate for a bit and come back.

I didnt read the novel but this is what i understood of the plot so far so theres no spoilers as i dont know whats gonna happen:

So far it seems that all or most of the characters used to live on another world where MC used to have a sister and maybe a boyfriend? The sister and her friend wrote a novel where MC committed suicide as revenge for a fight they had. Later Something seems to have happen in that world that we dont clearly know yet, but MC, the sister and friends all died. Then they all reincarnated inside the novel the sister and her friend wrote.

When MC reincarnated as a character in this novel (second world) he remembers having read the content of the novel before and nothing else, but slowly those memories started disappearing.
He now realized that his sister reincarnated in the character of his brother in the novel and she/he remembers the past. In a way she/he is also at fault of the tragic story of his character because she wrote part of the novel in the original world.

We dont know yet if ML used to be someone in World 1, but i heavily speculate he was probably the hinted boyfriend or crush MC used to have. But ML clearly doesnt have any memories of another life, so he just thinks that MC is having a psychiatric problem and wants to keep him safe because he loves him.

Also MC has all this medical issues that are probably linked to the reincarnation or what happened in their previous life. We are still waiting to know why this is happening to him.
