Too far gone to stop but still pissed at the doctor

Just realized why the execution is odd to me , the characters did not want this, tried to stop the pregnancy , but the damn doctor is like a Mcguffin that makes sure they get pregnancy plot, told them to have sex, no condom, no abortion allowed...like no natural obstacle..just get the doctor to make up a new rule
