I've read the spanish translations and there, instead of "A girl - on the outside, she may look like a doll. But on the inside she's like this?", it's something like "A woman is like a doll from the outside, but like a snake inside" and so he thinks she is "an unusual slave". Just thought I should point that out, I think it helps to better unsderstand his thoughts, I mean no disrespect for the translators!!! (:
