After she had the poison the knight changed Lindsay's shirt and Lindsay notice someone had unwrapped her chest. Unless the knight had someone else changed Lindsays shirt but i think it was inferred that the knight is the one who knows

Okay is it just me or is he def smart enough to know/recognize the girl who was with Leonard at that big library place? Especially seeing her "dress up" as a girl later. And i def cant see the knight not telling him Lindsay is a girl but ╮(￣▽￣)╭
