While Griffiths basically dropped Caska in order to enthrall Gatts, he also dropped both of them in order to seduce the Crown Princess Charlotte — a process which began as soon as they were admitted to the Court of the King of the Middlelands. Do you remember the conversation he had with Charlotte back along the corniche of the palace gardens where he repudiated all his followers and, basically, told her (or Gatts, actually, who was standing just under the ballustrade, eavesdropping, while the meaning of Griffith's speech was plainly sailing over Charlotte's head) that he despised anyone who gave up their dreams to follow his? Maybe that was intended as a sort of psychological kick in the pants to motivate Gatts to start showing some leadership, but Gatts is a very simple guy and, if there was an subtlety in Griffith's meaning, it didn't work as Griffith intended.

Gatts took it as a signal to move along, not to conquer Griffith. So he licked his wounds with Caska, defeated Griffith in their duel and moved on, setting the whole deflowering scene with Griffith and Charlotte in motion, and we know how that ended. When faced with the utterly maimed, crippled and disfigured Griffith who was rescued from the King of the Middleland's dungeons, Casca's compassion and gratitude meant that she told Gatts she had to remain with Griffiths and could not leave and follow him.
Griffiths, meanwhile, had lost none of his ruthlessness, and was still beholden to the power of the scarlet Behelit. When Griffith became the fifth Godhand, he left his remaining damaged physicality behind (or suppressed it) along with his soul. In doing so, he became trapped inside the Behelit/Astral Plane Hellworld and could no longer manifest in the physical realm. To materialize, he needed a physical body. That's why he raped Casca and that's why he allowed Gatts to rescue her from the river of the damned. The first thing she did when she returned to materiality was to give birth/miscarry that horrible deformed embryo/foetus thing. Gatts wanted to stop her, but she treated it like a beloved baby, and it so resembled the circumstances of his own birth, he couldn't kill it, even if it was clearly a monstrosity. He could perceive the sentience within it.

The little horrible embryo kept following them up until they ended up at the scene of the second eclipse, where that egg-shaped "Apostle"/demon gobbled it up in order to create his "new world". It ended its birth cycle in the body of that goblin and burst out, during the eclipse, into the reborn "Hawk of Light". Well, the Hawk of Light IS the fifth "God-Hand", that creepy thing which Griffith turned into when he sacrificed all his companions. He looks pretty, but he isn't human, just like Falconia looks like a beautiful refuge and City of Light, but is really a feeding ground for demons.
In short, the Fifth Godhand/Griffith raped Casca so he could be reborn as the Hawk of Light.
