While I do want to see their baby, I don’t want it to be an accident since he said he was scared to get pregnant. I think the end is symbolic with Heaven and Hell - kinda like how their mom said “down here is hell” & sinners staying down below while innocent people ascend to heaven. The brothers, who are bad people, stayed below.
The doctor was being pushed to change so that he could survive “down there”. We never see the doctor arrive to the top, so we don’t know if he “made it to heaven” or if he changed too much to be able to “make it in”.
While we don’t see the brothers die, the only thing certain is they stayed below/“stayed in Hell”. Unlike the doctors who may or may not have made it above.
