So in the novel, Leo notices that the princess is actually a boy but she already knows he is a boy because of the book she read before she transmigrated. She wanted to make sure that she remembered properly so she asked Pelio "why is Scandia wearing a dress even though he is a boy", which shocked him because it was kept as a tight secret that no one should know that the princess is actually a prince. Pelio eventually said yes and asked her how she knows that the princess is a boy but Leo just said that she knows how to distinguish the build of a man and a woman. Anyway, I think the only people who know that the princess is a boy is the Empress, Pelio, and Leo (and maybe a few others too, I haven't read that far in the novel yet, only the upcoming chapters). I think the empress doesn't want to reveal that scandia is a boy because of the possible power struggle of obtaining the throne. But as the book described (in Leo's memories), "Princess Scandia grew up to be a very good-looking muscular man", which Leo thought of happily because of the muscles (≧∀≦)
