I think it happened in the beginning. When he was fighting Agia I believe
When Agia was 12/13 years old his stepdad betrayed Agia and his mom when they tried to escape together. Agias stepdad tried to hand him to Verick and in the process Agias mom died. Verick tried to kill Agia and Agia turned into his original form and cut Vericks arm off.
