I like to think that Satoko actually did die and that what happened at the end is just Shinpei reuniting with her in their house since I have shed too much tears over this for it to be any different!!!
And in general I just think it makes more sense in a way since ever since the beginning no doctor could cure her illness or the cause for it, and now suddenly she's cured from her illness so to me it just kinda seemed like a forced happy ending???
