I understand why gang leaders (in fiction and in real life) are almost always violent, crazy-ass psychopaths. But why do people follow them? Me, I see nutjob gang leader coming my way, I'm making tracks in the opposite direction. Fast.

Because they make lots of money and gain lots of power much more quickly than washing dishes, selling shoes, or any number of normal jobs they would have otherwise wound up working in. Then there's the fact that they don't really have to follow the rules of most people, pay taxes, show restraint in any real sense, etc.
