Im not sure which part needs explaining, so ill just start from the beginning.

Takatora was the popular kid in his high school being the heir of the Kumajishi group. He was a rich alpha and the student body president, and dated Kurasuma, the most popular Omega in the school, but was very much a chauvanist and a misogynist who considers Omega's to be livestock that exist to make babies. However, he and his family just assumed he was an Alpha because of his appearance. But during class he goes into heat and finds out he is an Omega too. The school bullies try to SA him, but Arisaka who is labeled the school slut and prostitute saves him but gets attacked instead. Arisaka also SA's Takatora soon after. Because Takatora does not want to get pregnant at all, he has his uterus removed, and somehow or another, ends up dating Arisaka once his boyfriend dumps him since he isn't an Alpha.
Because he can't have kids, and he isnt an alpha either, Takatora's family sees no value in him, and disown him and kick him out of the hosue while hes still in high school. So his uncle Tsuruta takes him in. Takatora then is moved to the Omega class, which is way less rigerous in coursework, but it's where he meets the rest of the class.

Arisaka however has a terrible family situation as well, his mom is trying to marry him off to greezy old men once he graduates high school. So on the day of graduation, Arisaka runs away from home and dumps Takatora via ghosting. Only Kameyama knows his wareabouts. Kameyama comforts Takatora over his frustration of being dumped without warning again, and they become friends.

Skip a few months later, and in college, Kameyama and Takatora hook up one night, and end up becoming boyfriends, and move in together (it's very U-haul Lesbians vibes). Kameyama is studying law and politics to become a politician and change the system to give Omega's rights, and Takatora is a stem student trying to make it on his own since he doesnt have his parents support, by creating a tech startup with his uncle's guidance since he's business savy.
However one of the Alpha's on his team SA's him one night in the office. However Uncle Tsuruta gives him the terrible advice of "if he didn't rape you, then suck it up and be a man. It will be a funny story when you are older".
Then his 5 year relationship with Kameyama continues but spirals.

To cope with this Takatora gets obsessed with getting married and starting a family. It's something he wanted all along, but not to the extent he's acting now. Even though Kameyama accepts his marriage proposal, he isn't really ready to get married, and he doesn't want kids and let's Takatora know this.
Takatora who is very conservative in upbringing despite being gay, wants a traditional family unit, and asks Osuzume if he will be the serogate for him and Kameyama's children. Kameyama finds out about the serugoacy stuff, and is completely against surrogacy, deeming it as using a Omega's womb like its a product. Taketora argues that it's not and that he wants to have a kid with him because he loves him, stuff happens and Kameyama dumps Takatora.

While Takatora mourns the end of his engagement and 5 year relationship with Kameyama but binge drinking while watching sappy stuff on tv, his uncle takes advantage of the situation by getting him to agree to going on a match making date with an Alpha. His uncle thinks it will appear better if Takatora enters a marriage of aperance with an Alpha despite him being gay. When Takatora says he doesn't want to have a baby with anyone but Kameyama, Tsuruta tells Takatora that it's his duty to the family to have a baby no matter what. And yes, I hate this manipulating uncle tsuruta.

Takatora goes on the date with the Alpha, is miserable. And bails when he gets a text from Kameyama who explains that neither of their perspectives or wants in a relationship are wrong, but what they want in life is too fundamentally different for their relationship to work. So they leave on good terms wishing the best to each other.
However Takatora was still emotionally spiraling, so he immediately goes to Osuzume's house and asks him to marry him and have his kids. Takatora explains that he is suggesting a marriage of convenience, since Osuzume is gay too but also wants to be a traditionally feminine kind of trad wife but also needs money since his family is suffering, and Takatora wants to get married have kids and be like the Alpha in the relationship and has the money to support Osuzume and his family. And the idea is to just home they learn to love each other along the way like with an arranged marriage. Plus they are both very much expending gender dysmorphia on the opposite spectrums of each other, so this is also to affirm their gender, which tells you this marriage is going to be messy, since that is a big reason why they are getting married.

Times kid to now, Osuzume and Takatora are married, and Osuzume is pregnant via IVF with a sperm donor (Takatora's younger brother). Uncle tsuruta and the board decided to merge Takatora's Company with his parents company despite his disapproval. However Osuzume and Takatora's relationship is rocky to say the least, plus Osuzume's parents dont approve of it because they are homophobic.
