Game MC swapped places with Real Life MC when they were 9.

So Real Life MC was in the game for the next 20 years and built a clinic.

When 20 years passed, Real life MC swapped again with Game MC. Now they are in their right timeline.

Game MC was the one we saw that was playing games in Chapter 1 and seem to have no recollection that he swapped with Real Life MC when he was 9 so HE THINKS he is actually from the real world.

So when the 20 years later swapped happened again, the story narratives has been showing the Game MC up until his sacrifice. So Game MC is technically dead...? Chapter 61 is not showing the continuation but rather now the timeline and side of Real Life MC.

Chapter 61 is basically now showing what has happened with Real Life MC in real world after switching again with Game MC. Hence why the desk has lots of rubbish on it.
