As the other comment said, the true reason + a lot of the details are huge spoilers from chapter 300+… but it’s not a ‘fake world’, it’s more of an alternate universe where ghost stories are real. The suffering of everyone else is genuine, he can feel pain and if he died he wouldn’t return, he’d just be dead because he’s stuck somewhere thats not his home and can’t go back (yet). His real, original world is not affected by any of this, since they’re separate universes. Hopefully answered without spoiling!
