Their relationship starts off in a twisted and problematic way but it gets sorta healthier and cuter with time
Worth reading for the art alone even if the story isn't your cup of tea

There is references to incest just because of royalty. It's just a thing. But also the ML pretends to have had an incestuous relationship with his sister. So he states a couple times that they have, but it's a lie and nothing is shown.
There is also referenced incestuous infatuation from their other siblings, the king, who has the hots for the sister. Again, nothing is depicted.
