Geez J is not some kind of middle age man when they met. He was still a kid himself! That's why everyone was trying not to be attach to him/think of him as a human because imagine forcing the weight of the world to a child. Also, they might have been affection there but did they is it implied they started a relationship at that time? If anything Sayoung was attracted to Uijae before he was sure he was J and Uijae only view Sayoung the adult one before realizing sayoung was the kid he saved. Incest where? Adoptive where? Raised who?
I think it's more like sayoung liked him first (like a crush) and in time they spend together they got closer and J starts to like him in that way

J, the teen, used to go visit this 4 year younger kid because he was lonely af and this kid was also lonely af. they were basically less lonely having each other. however, there is a huge difference between rising someone and going to visit that same someone at the hospital whenever you have the time. They were solace to each other. They were becoming found family. In no way did J rise ML.

J was teetering on 20, yes, but there were never romantic undertones during their time pre-WSR. They were two lonely people who found comfort in each other's presence, nothing more. The years they spent apart are quite literally what brought them together.

J had taken care of LSY only for a few weeks/months and saw him as a brother. If J kept taking care of him and never went in the rift, their relationship WOULD stay as brothers (which is what happened to Timeline 1 and Timeline 2).
However, too much time had passed after J went in the rift. There was never a familial connection between them. Even LSY mentions this! They meet again without knowing each other's identities and developed a sweet, vague relationship so J, as much as he tries, admits to not being able to see LSY only as "the boy he saved and is responsible for".
In the next chapters, LSY addresses this. J currently doesn't know how to treat LSY (like the kid or like the asshole he is) so LSY bluntly says "I want an equal relationship, J."
