Usually, it's between two nobles, or a servant and a noble. (I guess servants are just seen as side characters in most cases). I think it's neat.
You have to be 110lbs in order to donate blood, and you can't do any strenuous activity afterwards to avoid dehydration. The drama is gonna be intense.
