It's called cauliflower ear and it happens to wrestler's when they get repeatedly hit.
That usually happens when you fight. Getting repeated punches to your ear fills it with liquid that gets trapped in there. You can search it up, you can see it on most boxers or mma fighters

They are cauliflower ears! Boxers like JJK often have those due to punches landing on the side of the face or on the ears itself. Which cause the blood vessels in the ear to rupture. Causing the ear to swell up with with blood(๑•ㅂ•)و✧
