There are times when I'm in a dream and I'd be questioning if this is in fact a dream or reality. So to get to the bottom of that, I'd pinch myself, but I'd feel the pain in the dream so reply
Interesting question. Now this got me wracking my brain. Because I lucid dream a lot and tend to be aware I'm dreaming... I don't recall feeling pain in dreams. At least, not anything worth noting. I think it depends. When I want to wake up in a nightmare, I just bite my arm and I don’t feel the pain, but the one instance I felt a little pain when I bite my hand is when I had a sleep paralysis reply
i have a nightmare that's been recurring over the last few months or so and what wakes me up from it is actually the fact that my head is aching so hard I can no longer stand to remain in the dream. afterward i have a pounding, almost migraine-like headache for a while that eventually fades. that's anecdotal but it's 100% my true experience, so mak...... 1 reply
As far as I know—though I’m in no way a professional—pain in dreams is considered rare and an indication of poor mental health.
How “real” the pain feels more so depends on the person, but not much is known due to how difficult dreams are to study.

This was my exact question when I got nightmares that feels real. Someone killed me and peel off my skin like I can feel the blade penetrated my skin and and hurts like hell.
