I think Toshio is more of a semi-immortal in a sense that he won't age physically, but his other biological parts definitely will and he will die. The scientist referred to him as a *lump of iron and meat* so he definitely have biological parts.
You know the saying "Your scientists were so preoccupied with whether or not they could that they didn't stop to think if they should."
I think that these researchers are somehow vaguely aware of this that's why they kept on telling Shindoh that *it's not human*, to convince themselves that they're doing these awful things to a robot and not a real person. They've become a hive mind.
Anyway, there are clues on what these human parts of Toshio are:
- Limbs (Muscles). Uncle mentioned they *broke* his bones, so maybe his aren't real bones anymore. He said it was so he won't break easily, but I bet it was also to stop him from growing. Because they broke his bones and replaced it, he won't get taller or maybe won't develop muscles. Then his muscles remained in his body so he would look EXACTLY the same except for his left arm to dehumanize Toshio.
- Brain. While his skull may be artificial with those additional ears attached, brain is definitely a real one. It's mentioned that they wiped his memories and only left enough cognitive function for him to understand basic commands. As we can see, his brain is still functioning/ improving, he dreams, learns, remembers events, and he even recovers a memory of his sister from years ago.
- Mouth, esophagus. I think they didn't modify this part because of you know what.
- Vocal chords. He needed to make sounds
- HEART. HELL YEAH. The way his heart pounds because of Shindoh or when he's happy. Nothing was mentioned about an artificial heart. But maybe they modified parts of it so he can handle extraneous activities. I mean, why would he fall in love so suddenly when he's been in this situation for years? When the uncle mentioned all his cognition and understanding is on the most basic level? When he's been with so so many men? Why did he only SINGLE OUT Shindoh out of all the people he interacted with? Why? Could it be that he recognized what kindness is and liked that kindness so much? Because he has a living, beating heart? I think so. Also I firmly believe the uncle is lying when he told Shindoh that Toshio can *take it 24/7*. Toshio sleeps whenever his activities at the facilities are done and at night while living with Shindoh. He even has dreams/ wet dreams. He does get tired but everyone doesn't want to consider a "robot" feeling tired.
- Nerves. I even think this is heightened so he'd react more sensitively for those goddamn buyers. He can feel anything he touches/ touches him. He can feel sensations and temperatures. Evident when he sweats whenever it's hot and shivers when he slept in the living room during winter. He even describes how hot the bath is or how peppermint makes his skin cool down.
- Eyes and tear ducts. He tears up (when he saw his sister) and rubs his eyes whenever he wakes up
- Blood and blood vessels. His skin flushes, his privates even reacted when Shindoh touched him in ch 1.
Parts that aren't/ may not be biological - Bones, teeth, hair, nails, anus/ rectum (it self-lubricates, wtf), stomach (He doesn't feel hungry so maybe they removed it making the digestion happen at his small intestines, and because they removed the excretory function, all nutrients go to his remaining human functions), urinary bladder, skin (Though he blushes and sweats, his appearance doesn't change. So his skin may be artificial, made to always look smooth and withstand wounds or rough clients), Lungs (possibly made stronger so he won't run out of air easily).
All in all Toshio is Wolverine. Jk, he's a sentient human-cyborg who is doing his best to live happily.

If you observe further, his cognition drastically improved when he's with Shindoh. Not only because Shindoh treats him like a human but because he began eating food again. I doubt people fed him in the facility.
