He is saying he has never had sex with any other woman in his entire life, but what he's saying is that he has not had sex with any other woman within the timeframe that would allow his niece to be his daughter.
If you pay attention to the scene, the empress is mad that he had sex with a woman besides her *after* she "bought" him, because he's "hers". That would be *after* Enoch's mother had passed.
The empress very likely knows about Genas's relationship with Enoch's mother, since he cares so much for her son and it was already widely known that he leaves her bedroom in the mornings.
He's saying in that scene, "to have a child I would had to have had sex with someone else while I was with you, which is impossible".
Also, I want to point out that even entertaining that he were talking about "in his entire life", he said that after being cut by her... Why would I believe him telling the crazy bitch that rapes and abuses him all the time that he hasn't had sex with Enoch's mother when there was a whole scene showing that they had? Enoch literally has an awkward spotting of it happening, drops his book running away, and then asks Genas if he loves his mother, who says "what's the point of resisting?" because she's in a higher position and her insanity has fully consumed her into thinking Genas is her husband/treating him as her husband.
I also want to remind anyone it concerns that, initially, we had no reason to think their relationship progressed to sexual, that was a sad twist that was confirmed in the scene with Enoch as a child. The story is vague at first, it shows her starting to lose herself and hallucinate and at first it looks like he's just taking care of her, only for it to then be hinted that he *might be having sex with her* with the rumors of him leaving her room in the mornings and then revealed he *is* when Enoch catches them together and runs away, only for it to *then* be revealed that he's not really willing, he just doesn't feel like there's any point in resisting. He's powerless at the time and she's the empress. Plus he feels sorry for her.

It starts off vague, with Letina still having some of her sanity. Enough to talk with her son sometimes. She ends up fully thinking Genas is Eben.

So again, the Empress is raping a rape victim that is traumatized by sex and doesn't want women touching him. At least with Letina it was more nuanced, but with bitch it's just one sided entirely and abusive to the max.
